In order to meet with a lay counselor, the Intake Assessment form needs to be completed. You may either pick a packet up from the front desk at the church office Monday-Thursday 9 am – 5 pm.

The Process
• Complete and sign intake assessment.
• Return completed assessment to church office Attention: Susan Leonard.
• You’ll receive a call to schedule your first appointment (Calls are returned Tues.-Thurs.).
If you need additional information on individual counseling, please contact us either by completing the confidential form below or leaving a message at 577-1604 ext 204.
Calls and emails are returned Tuesday thru Thursdays.
*Only the Director of Counseling and/or the Intake Lay Counselor will see and respond to your request. This is a completely confidential ministry.
